Summary
A 75-year-old female is experiencing difficulty breathing in the post-anesthesia care unit after an open colectomy. She has a history of congestive heart failure. Estimated blood loss was 250 mL. Total intravenous fluids administered were 4.5 L of Lactated Ringer’s throughout the three-hour case. She is breathing 34 times per minute and appears uncomfortable.

Pomatomus saltatrix is a high-value marine pelagic coastal fish, that is fished throughout subtropical and temperate coastal waters around the world. Despite its large economic potential, there are no global data on its genetic diversity, which could compromise the conservation of the species. The aim of this study was to analyse the genetic-evolutionary structuring of the species, with the intention of evaluating different genetic P. saltatrix stocks that may indicate potential species. Based on 157 Cytochrome C Oxidase Subunit 1 sequences, the molecular delimitation analyses of species (distance and coalescence methods), as well as the haplotype network, found profound geographic structuring related to five distinct units with high and significant FST pairwise values. The divergence of these molecular units is mostly related to the Pleistocene glacial and interglacial cycles of climatic oscillations. It is hypothesized that one ancestral lineage, adapted to cold water environments, diversified into two lineages, with one more adapted to warmer environments. The high values of global genetic diversity (π = 0.016; h = 0.96) may be related to the existing profound genetic differentiation. Due to the presence of five Molecular Operational Taxonomic Units (MOTUs) within the species it is necessary to employ different regional management strategies. Traits of low haplotype richness and shallow population contraction were identified in the MOTUs V (Venezuela and Brazil) and III (Turkey and Australia), respectively, representing conservation priorities. Other molecular markers, as well as morphological data, should be explored with the aim of defining the taxonomic status of P. saltatrix stocks.
How Much Do Young Italians Know About COVID-19 and What Are Their Attitudes Toward SARS-CoV-2? Results of a Cross-Sectional Study

Objectives:
At the end of 2019, an outbreak of novel coronavirus pneumonia, called severe acute respiratory syndrome coronavirus 1 (SARS-CoV-2), was first identified in Wuhan, Hubei Province, China. It subsequently spread throughout China and elsewhere, becoming a global health emergency. In February 2020, the World Health Organization (WHO) designated the disease coronavirus disease 2019 (COVID-19). The objective of this study was to investigate the degree of knowledge of young Italians about COVID-19 and their current attitudes toward the SARS-CoV-2 and to determine if there were prejudices emerging toward Chinese.
Methods:An online survey was conducted on February 3, 4, 5, 2020, with the collaboration of Italian website “Skuola.net”. Young people had the opportunity to participate by answering an ad hoc questionnaire created to investigate knowledge and attitudes about the new coronavirus, using a link published on the homepage.
Results:A total of 5234 responses were received, of which 3262 were females and 1972 were males. Most of the participants showed generally moderate knowledge about COVID-19. Male students, middle school students, and those who do not attend school, should increase awareness of the disease; less than half of responders say that their attitudes toward the Chinese population has worsened in the last period.
Conclusions:Global awareness of this emerging infection should be increased, due to its virulence, the significant risk of mortality, and the ability of the virus to spread very quickly within the community.

Objectives
The consumption of ultra-processed foods (UPF) has been linked to an increased risk of cancer in western populations, particularly for breast cancer (BC). Although the consumption of UPF is increasing rapidly in Latin American (LA) populations too, there has been no evaluation of its association with BC among LA women, where 20% of the BC cases present at ages younger than 45 years. We therefore evaluated the role of UPF intake on BC risk in young women participating in the Latin American PRECAMA study.
MethodsThe PRECAMA study is an ongoing study coordinated by the International Agency for Research on Cancer (IARC) and involves scientific teams in four LA countries (Chile, Colombia, Costa Rica and Mexico); 406 incident BC cases and population based controls aged 20–45 years are recruited. Trained nurses administered a lifestyle and food frequency questionnaires (FFQ), conducted anthropometric measurements and collected biological samples. Tumour receptor status are determined in a centralized laboratory. UPF consumption was estimated by applying the NOVA classification to the dietary consumption data obtained via the FFQs. The association of the consumption of UPF and BC was determined using conditional logistic regression analysis adjusting for potential confounding factors.
ResultsThe median age at recruitment was 40 years. The median percentage of calories from UPF was 24% (10th to 90th percentile: 9 to 43%). UPF intake was positively associated with BC risk (OR for tertiles 2 and 3 in comparison with tertile 1 were 1.84 (95% CI 1.21 -2.81) and 1.69 (95% CI 0.99 -2.89) respectively). Among the UPF most strongly associated with BC, we identified the intake of industrial bread, packaged sweet and savoury snacks, breakfast cereals, cakes and desserts, and ready-eat/fast food. Comparing the 3rd with the 1st tertile of energy intake from the UPF group “fast food” the OR for BC risk was 1.93 (95% CI 1.23–3.04).
DiscussionOur results show positive associations between the consumption of UPF and BC risk in young women in LA. Therefore, global actions to decrease UPF intakes are urgently needed to address the cancer burden. In addition, further studies are needed to disentangle mechanisms relating UPF intake and carcinogenic processes in the breast. The European Prospective Investigation into Cancer and Nutrition (EPIC) study, a large-scale cohort including ~500,000 participants and 18,814 BC cases will be used to unravel the mechanistic pathways underlying these positive associations between the consumption of UPF and BC risk.
DNA barcoding reveals cryptic diversity and peculiar phylogeographic patterns in mojarras (Perciformes: Gerreidae) from the Caribbean and South-western Atlantic

The mojarras (Eucinostomus) are a widespread group of coastal fishes of controversial taxonomy because of similarities in their external morphology. In the present study, we assessed the genetic diversity of species and populations of Eucinostomus using DNA barcodes using a systematic and phylogeographic context. In total, 416 COI sequences of all valid Eucinostomus representatives were analysed based on public databases and collected specimens from the north-eastern coast of Brazil (Western South Atlantic). Several cases of misidentification were detected in the barcode dataset (E. argenteus, E. harengulus, E. gula, E. dowii and E. jonesii) that could account for the taxonomic issues in this genus. In contrast, we identified four molecular operational taxonomic units (MOTUs), with divergence above 2% in the Western Atlantic, that correspond to cryptic forms within E. argenteus, E. harengulus, E. gula and E. melanopterus. These data suggest that Plio-Pleistocene events (rise of the Panama isthmus, Amazonas outflow and sea-level fluctuations) played a major role in the diversification of mojarras. While subtle morphological differences have been used as proxies to discriminate Eucinostomus species, the genetic data proved to be efficient in differentiating them and revealing potentially undescribed taxa. Therefore, we recommend that further taxonomic studies in mojarras should incorporate DNA-based evidence.
Reliability and validity of an FFQ for South American children and adolescents from the SAYCARE study

Objective:
The purpose of this study was to analyse the reliability and validity of a semi-quantitative FFQ to assess food group consumption in South American children and adolescents.
Design:The SAYCARE (South American Youth/Child cARdiovascular and Environmental) study is an observational, multicentre, feasibility study performed in a sample of 3- to 18-year-old children and adolescents attending private and public schools from six South American countries. Participants answered the FFQ twice with a two-week interval and three 24-h dietary recalls. Intraclass and Spearman’s correlations, weighted Cohen’s kappa (κw), percentage of agreement and energy-adjusted Pearson’s correlation coefficients were calculated.
Setting:Seven cities in South America (Buenos Aires, Lima, Medelin, Montevideo, Santiago, Sao Paulo and Teresina).
Subjects:A sample of 200 children and 244 adolescents for reliability analyses and 252 children and 244 adolescents for validity analyses were included.
Results:Depending on the food group, for children and adolescents, reliability analyses resulted in Spearman’s coefficients from 0·47 to 0·73, intraclass correlation coefficients from 0·66 to 0·99, κw coefficients from 0·35 to 0·63, and percentage of agreement between 72·75 and 83·52 %. In the same way, validity analyses resulted in Spearman’s coefficients from 0·17 to 0·37, energy-adjusted Pearson’s coefficients from 0·17 to 0·61, κw coefficients from 0·09 to 0·24, and percentages of agreement between 45·79 and 67·06 %.
Conclusion:The SAYCARE FFQ achieved reasonable reliability and slight-moderate validity for almost all food groups intakes. Accordingly, it can be used for the purpose of ranking the intake of individuals within a population.
Factors associated with stunting and overweight in Amazonian children: a population-based, cross-sectional study

Objective
To examine the prevalence of stunting and overweight in children and identify demographic, socio-economic and maternal characteristics, as well as biochemical indicators, associated with these outcomes.
DesignA population-based, cross-sectional study was performed. Data from structured questionnaires, anthropometric measurements, and blood and stool samples were used in Poisson regression models to estimate prevalence ratios (PR) according to a hierarchical conceptual framework.
SettingAcrelândia, western Brazilian Amazon.
SubjectsChildren (n 1139) aged <10 years.
ResultsPrevalence of stunting was 7·1 % (95 % CI 5·1, 9·6 %) and 3·7 % (95 % CI 2·4, 5·7 %) among children aged <5 years and ≥5 years, respectively; overweight was detected in 20·6 % (95 % CI 17·4, 24·2 %) and 9·4 % (95 % CI 7·2, 12·1 %) of children aged <5 years and ≥5 years, respectively. Among children <5 years of age, stunting was positively associated with the lowest maternal height tertile (PR = 3·09, 95 % CI 1·26, 7·63), low birth weight (PR = 2·70, 95 % CI 1·41, 5·19), diarrhoea for ≥3d (PR = 2·21, 95 % CI 1·03, 4·77) and geohelminth infections (PR = 2·53, 95 % CI 1·02, 6·13). Overweight in children <5 years of age was positively associated with caesarean delivery (PR = 1·45, 95 % CI 1·02, 2·06), birth weight ≥3500 g (PR = 1·82, 95 % CI 1·30, 2·55) and Fe deficiency (PR = 1·64, 95 % CI 1·07, 2·53). Among children aged ≥5 years, land or livestock ownership (PR = 1·85, 95 % CI 1·07, 3·22), maternal overweight (PR = 2·06, 95 % CI 1·23, 3·47), high C-reactive protein concentration (PR = 2·43, 95 % CI 1·26, 4·70), vitamin A deficiency (PR = 1·97, 95 % CI 1·13, 3·41) and high serum TAG concentration (PR = 2·16, 95 % CI 1·27, 3·68) were associated with overweight.
ConclusionsOverweight was more prevalent than stunting, being associated with higher household wealth, maternal overweight, caesarean delivery, high birth weight, micronutrient deficiencies and high TAG concentration. Improvements in maternal and child health care with sustainable access to healthy food are necessary to reduce short- and long-term health complications related to overweight in this population.
Anaemia and iron deficiency between 2003 and 2007 in Amazonian children under 2 years of age: trends and associated factors

Objective
To describe trends in the prevalence of anaemia and Fe deficiency in children under 2 years of age living in a town in western Brazilian Amazonia.
DesignTemporal analysis of two cross-sectional population-based surveys. Information on socio-economic status, morbidity and breast-feeding was obtained using a structured questionnaire. Child weight and length were measured for anthropometric evaluation. Concentrations of blood Hb, plasma ferritin and soluble transferrin receptor were measured.
SettingThe town of Acrelândia, state of Acre, north-west Brazil.
SubjectsA total of 170 and 224 participants of the 2003 and 2007 surveys, respectively.
ResultsComparison between the 2003 and 2007 surveys revealed no statistically significant differences in the prevalence of anaemia (48 (95 % CI 39, 56) % to 40 (95 % CI 33, 47) %) or Fe-deficiency anaemia (39 (95 % CI 30, 48) % to 37 (95 % CI 30, 45) %), respectively. However, an increase in the overall prevalence of Fe deficiency from 62 (95 % CI 51, 68) % to 81 (95 % CI 75, 86) % was observed (χ2 test, P ≤ 0·001). In age- and sex-adjusted analyses for risk of Fe deficiency, only early introduction of cow's milk (<90 d) was associated with Fe deficiency in 2003 (prevalence ratio (PR) = 0·76; 95 % CI 0·57, 1·01), while caesarean section (PR = 1·18; 95 % CI 1·03, 1·35) and birth weight <3500 g (PR = 1·15; 95 % CI 1·00, 1·34) were associated with Fe deficiency in 2007.
ConclusionsNo improvements were observed in the prevalence of anaemia, exposing a worrying scenario for public health, while a significant increase was found in the prevalence of Fe deficiency in the studied infants and toddlers.
